Diabetes Workforce Training: Diabetes Education and Management


Description
This training presentation provides an overview of diabetes education, prevention, and management to support effective patient education and care coordination. Participants will review the differences between Diabetes Self-Management Education and Support (DSMES) and the Diabetes Prevention Program (DPP), the role of Certified Diabetes Care and Education Specialists (CDCES), and the seven evidence-based diabetes self-care behaviors. The training also addresses the impact of social drivers of health on diabetes outcomes and introduces community resources, including the Diabetes Awareness & Wellness Network (DAWN). Through interactive activities and real-world scenarios, staff will strengthen their ability to identify patient needs, reinforce self-management strategies, and connect individuals to appropriate prevention and education services.
